Reviewing and signing a service agreement

This guide is for a participant. Read the agreement's supports, prices and terms before signing. You can take your time. A supporter reading with you does not gain authority to sign in your place.

GST means goods and services tax. The signing form asks you to confirm statements used for the agreement's GST treatment. If you do not understand a statement, ask before ticking it.

Before you start

Your recorded plan needs dates and funded categories for the plan confirmations. See Your plan and budget.

Check Settings, then My details, if the agreement asks for a missing NDIS number or date of birth. You, or a permitted plan nominee, can add missing values. Check carefully before saving: support must help correct a saved value. See Privacy and security.

Read and sign on the web

  1. Open Review agreement from What needs me, or select the waiting agreement in My agreements.
  2. Read all eight sections. The view shows the saved agreement terms, including the actual prices and cancellation terms. If anything is missing or differs from what you discussed, ask the worker before signing.
  3. Reach the end and select Continue to sign. Jump to the end moves the page; it does not mean you have read or accepted it.
  4. On Four things to confirm, tick each statement only if it is true:
    • My NDIS plan is current.
    • These supports are in my plan.
    • This agreement is in writing.
    • The supports are the kind GST-free rules cover.
  5. Select Confirm when all four are available and ticked.
  6. If a plan-manager invoice-sharing question appears, read it and make your own choice. An unavailable automatic-sending control does not authorise or send anything.
  7. Type your full legal name exactly as your account records it on Sign your agreement.
  8. Select Sign and wait for the result.

A permitted nominee types their own name when signing for you. A correspondence-only role cannot approve financial matters.

On the native phone app

Open Your agreements and choose Read the agreement or Read the change. Read the actual terms and prices. Check what is included explains the required agreement parts; it does not sign them.

If your role and the current record allow signing, tap Sign this agreement, enter your own full name on Sign your agreement, then tap Sign now. Signing requires a connection. The native flow differs from the web's four-tick sheet, so follow the controls it actually presents. Check the signed result and copy before leaving.

Check the result

A confirmed signature appears in My agreements. If the worker still needs to sign, the agreement is not active yet. Once both required signatures are recorded, check Signed and active and download the PDF when it is ready. It is also available in My documents.

Changes to a signed agreement use a new written variation. Read a proposed change before signing it; the earlier agreement stays in the record.

If something goes wrong

  • A plan statement is grey. Recorded dates or funded categories are missing. Add or check the plan information before trying again.
  • Missing participant profile details. Use the link to My details to add them, then return to the agreement.
  • The name was refused. Use the full legal name recorded for the person signing.
  • You are unsure. Go back without signing and ask your worker.
  • Signature was not confirmed. Reopen My agreements before retrying. Do not assume a filled name box is a saved signature.
